1. Purpose. Willowbrook Senior Aid coordinates volunteer visits and benefits assistance for older adults in Willowbrook County.

2. Information. You agree the information you provide is accurate to the best of your knowledge.

3. Volunteers. Visits are performed by screened volunteers. You may request a different volunteer at any time.

4. Sharing. Your request is shared only with the volunteer assigned to you and with our program coordinator.

5. Retention. Requests are kept for three years, then destroyed.

6. Cancellation. You may cancel a scheduled visit by calling our office at least 24 hours in advance.

7. No medical care. Volunteers do not provide medical or personal care services.

8. Acceptance. Checking the agreement box on the request form confirms you have read all of the terms above.
