for public benefits. These terms govern your use of the application.
2. Information you provide. You agree the information you
submit is accurate to the best of your knowledge. Knowingly providing false
information may make you ineligible.
3. Sharing. We share your application with the agencies
administering the benefit you select, and with no one else.
4. Retention. Applications are retained for seven years
as required by the administering agencies, then destroyed.
5. Your rights. You may request a copy of your application
or ask us to correct it at any time by contacting our office.
6. No guarantee. Submitting an application does not
guarantee you will receive benefits. Eligibility is determined solely by the
administering agency.
7. Assistance. If someone helped you complete this form,
they are not responsible for the accuracy of the information you provided.
