Extended-release monthly injection (Sublocade) 100 mg or 300 mg for maintenance
Eligibility
Adult (18+) with documented or self-reported opioid use disorder
Willingness to attend counseling and peer meetings for the first 90 days
No specific requirement for prior treatment attempts
Chronic pain patients may be evaluated on a case-by-case basis
