Buprenorphine Program

Same-week induction, ongoing counseling, and long-term maintenance for opioid use disorder.

Program Structure

• Days 1–3: Assessment and induction (in-person or telehealth)
• Weeks 1–4: Weekly clinical visits to titrate dose — also weekly peer meetings
• Weeks 5–12: Bi-weekly clinical visits — ongoing peer meetings and counseling
• Beyond 12 weeks: Monthly maintenance visits, quarterly urine toxicology, ongoing behavioral health as clinically indicated

Medication Options

• Sublingual film (Suboxone) 2/0.5–16/4 mg dose range
• Sublingual tablet (Zubsolv) 1.4/0.36–11.4/2.9 mg dose range
• Extended-release monthly injection (Sublocade) 100 mg or 300 mg for maintenance

Eligibility

• Adult (18+) with documented or self-reported opioid use disorder
• Willingness to attend counseling and peer meetings for the first 90 days
• No specific requirement for prior treatment attempts
• Chronic pain patients may be evaluated on a case-by-case basis

Program Outcomes (2023)

• 184 participants inducted
• 142 (77%) actively engaged at 6-month mark
• 67% report no illicit opioid use at 6-month follow-up
• 0 overdose deaths among active participants
